How to Submit a Claim

If you need to submit a manual claim for reimbursement, you can do so by selecting one of the following procedures.

  • You can submit a paper claim for reimbursement.

Mail your form and the required receipts and/or Explanation of Benefits Form to the following address:

  • Fax your form with the required receipts and/or Explanation of Benefits Form to (610) 853-9075.
  • Email your form with the required receipts and/or Explanation of Benefits Form to service@theharrisongrouponline.com
  • Electronically file your claim online by logging into your account and following the claim filing instructions.
